Assisted Living
We have three Assisted Living apartments, which allow for independence with the security of knowing that medical help is just down the hall.

Rehabilitation Stays
We provide post-acute care to those recently hospitalized.

Pine Ridge prides itself on our rehab-to-home program, which allows local residents to receive inpatient therapy in Pagosa Springs, CO. The focus is to allow people to safely return home and regain independent function in their homes.

Nursing Home
We are proud to be a place where residents can live and feel at home while still having the security of nursing care 24 hours a day.  Pine Ridge has complete services available to provide health care for both short and long term stays.  We utilize the team approach in coordinating health care while maintaining a nurturing home like environment for our residents.

End of Life
Reaching the end of life is often difficult for families and friends. At Pine Ridge we not only stress quality of life, but dignity in death. We contract with Hospice to provide not only physical comfort to the resident, but emotional support to the family and friends.
