When you enter Pine Ridge, you notice that this is not your typical nursing home. The atmosphere is light and eloquent and the only odor might be the smell of fresh popcorn or homemade cinnamon rolls.
But more than the relaxing atmosphere, what visitors see is the residents involved in their lives. Although we do have some residents who are very ill or disabled, most of the people living here are active and interested in the world we live in. They love to catch you and talk about their interestes and past memories. We have been fortunate to have many groups from the community come and spend time here and the resounding comment is, "What a great group of people live here!"

Pine Ridge Care
Pine Ridge is a special place that has been thoughtfully planned to address both long and short-term medical needs of people through skilled nursing care. Pine Ridge offers appropriate care for all ages and needs. We have three Assisted Living Apartments which allow for independence with the security of knowing that medical help is just down the hall.
Our nursing staff is superior in this field! RNs are available 24 hours a day, along with LPNs, and certified nursing assistants. Many compliments are received from family members for our compassionate care to residents and their families, especially during difficult times when residents are acutely ill or nearing the end of their life. We take pride in providing outstanding care and nurturing to our elders creating happy memories.
We accept Medicaid and Medicare which provides post-acute care to those recently hospitalized. We pride ourselves on our rehab-to-home program, which allows local residents to receive inpatient therapy in Pagosa. The focus is to allow people to safely return home and regain independent function in their homes.
Reaching the end of life is often difficult for families and friends. At Pine Ridge we not only stress quality of life, but dignity in death. We contract with Hospice to provide not only physical comfort to the resident, but emotional support to the family and friends.
To meet our resident's spiritual needs, The Pagosa Ministerial Alliance provides weekly church services in the facility, denominations rotate Sundays. Volunteers from the local churches provide communion and Bible study for our residents.

Staff
We have a wonderful staff who are committed to the people they care for. At present, we employ 45 full time and part time employees.
Mary Brooks, our Administrator, moved from Texas in 1998 and has worked hard to develop a team of professional supervisors. This allows the staff to care for our residents in a consistent and individualized manner.
Jim Pruitt, MD, is our Medical Director. His goal is to assure that the resident's medical needs are met in a timely, appropriate and compassionate manner.
Linda Mozer, RN, is our Director of Nursing. She has the task of organizing and planning the medical care for the resident. She is a highly skilled professional nurse who is committed to quality of care.
Andrea Constant, LPN, is our Admissions Coordinator. She coordinates admissions and plans any discharges. She also works hard to see that each resident and family's emotional and psychosocial needs are being met.
Mary Ann Martinez, our Activity Director, has a calendar packed with entertainment, games, crafts, music, outings and social events. The sky is the limit for what we can do for fun. You can see our bus around town attending parades, plays, concerts and other community functions.
Jim Garcia, Food Service Supervisor, has the difficult task of providing homelike meals that meet nutritional guidelines for our elderly. She never hesitates to go the extra mile to provide a special meal for our residents and staff.
Brenda Carmickle is our Administrative Assistant. She handles the numerous incoming calls and manages personal need accounts for our residents. She often will write and send greeting cards to those residents who never receive mail.
John Snarr is our Maintenance Director. He oversees the grounds and keeps the building in excellent condition. He also takes residents to appointments and makes the trips fun!
Nancy Williams, Medical Records Director, works closely with the doctors to assure that the charts are accurate and in good order. She is quick to help the nursing staff and arranges appointments.
Sabina Archuleta, Environmental Supervisor, oversees the facility to keep it attractive and odor free for residents and visitors. She also oversees the laundry.
We also employ a bath aides, registered nurses, licensed nurses and certified nurses aids.

History and Management
Pine Ridge Extended Care Center opened the doors to our first resident on November 4, 1997. The start was slow as people learned of the facility and began to inquire about our care. In less than three years, we filled the facility to almost capacity, with wonderful residents full of humor, stories and life.
Pine Ridge has been owned by the same group of board members and investors who started the project, and their commitment to this building and the people who live and work here has been astounding. Pine Ridge is managed by C&G Management from Mancos, CO.
Our 24-hour professional staff works together as a team to provide quality care for our residents and loved ones. We invite you to come visit our home!
